Upon completing all the nomination procedures electronically, approving declaration, pledging forms and filling in the personal information and other information required for admission to the Health track, you will be able to print the Admission Slip, which includes the Professional Fitness Disclosure Form.

• - The form contains your Full name, your national ID/Iqama number, and your mobile number. You must keep it until the appointment of the examination.
• - You do not have to come to the Deanship of Admissions and Registration for any point, just keep the form until an SMS message arrives, specifying the appointment of the medical examination, knowing that the examination is for those who are admitted in the (Health Track, Health Path of the College of Applied Medical Sciences in Jubail and Nursing Track ) only.
• - In your appointment, go to Family and Community Medicine Center (FAMCO) and bring with you (4x6 personal photo + National ID and a copy of it).
• - FAMCO will have a committee formed to welcome the candidates and facilitate the procedures for their medical examination.
• - The center will perform the examination procedures and send the results to the Deanship of Admissions and Registration later, and you do not have to bring it or a copy of it to the Deanship of Admissions and Registration.
